Ciavarra Elementary School will host Kindergarten Round-up for 2018-2019Tuesday and Wednesday, May 22 and May 23 at the elementary office.
Parents are asked to please bring the child who will be in kindergarten to registration. Our kinder teachers want to meet him/her and give a very quick readiness assessment.
On Tuesday, May 22, the time for registration is 4-6 p.m. and on Wednesday, May 23 the scheduled time is 1:30-4:30p.m.
Parents who plan to register their children for the 2018-2019 kindergarten class should have the listed documents with them. They will complete an enrollment packet at the time of registration.
Documents required:
• Original birth certificate
• Child’s Social Security Card
• Photo ID of parent/guardian with current address
• Proof of residency within the boundaries of Devine ISD, which must include one of the following:
–Rental Agreement or
–Lease Agreement or
–Property Tax Receipt / Mortgage Agreement or
–Multiple Family Residence statements (Forms available at registration.)
• Immunization Record
Vaccinations required:
–DTP/DT/Dtap—4 or 5 doses*
–IPV—4 doses **
–HIB—4 doses
–MMR—2 doses
–Hepatitis B—3 doses
–Hepatitis A—2 doses
–Varicella—2 doses
*5 doses of Dtap; one dose must have been received on or after the 4th birthday. Four doses meet the requirement if the 4th does was given on or after the 4th birthday.
** 4 doses of polio; one dose must have been received on or after the 4th birthday. Three doses meet the requirement if the 3rd does was given on or after the 4th birthday.
Children will not be placed in a class until all the above documents are complete.
